This is a proactive measure. You must PROVIDE the summary. This means you must give it to the patient either in paper or electronic format. Electronic format can be a cd, flash drive or uploading the information to the patient portal. Just offering it is not enough and you can not wait for the patient to ask for the clinical summary.
must respectfully disagree-according toCMS guidelines in a Q&A seminar-CMS respnded that it is proactive to offer the summary-if the patient declines the summary ,you do not have to produce it. Offering it is enough-it does not have to be printed out if the patient does not want it.
