Seeing Toenail Fungus Differently: The Role of Stain in Visualizing Fungal Involvement

7:00 - 8:00 PM Eastern
Accurate diagnosis of onychomycosis remains a challenge in podiatric practice, particularly when clinical findings overlap with other causes of nail dystrophy or conventional laboratory testing produces inconclusive results. This lecture will review current diagnostic approaches to onychomycosis and introduce the use of 0.5% methylene blue staining as a potential inexpensive point-of-care adjunctive tool for visualizing fungal involvement in the nail plate. Findings from a clinical study evaluating staining patterns in 136 toenails will be presented, with emphasis on the relationship between persistent staining and laboratory-confirmed infection. Practical applications, interpretation of staining results, limitations of the technique, and its potential role in clinical decision-making will be discussed.

Learning Objectives

Upon completion of this lecture, participants will be able to:

  1. Identify the diagnostic challenges associated with onychomycosis and describe the limitations of conventional diagnostic methods, including KOH microscopy, fungal culture, and PCR.
  2. Describe the principles and clinical application of methylene blue staining, including the interpretation of positive, negative, and inconclusive staining patterns in suspected onychomycosis.
  3. Evaluate the clinical evidence supporting methylene blue staining as an adjunctive diagnostic aid and discuss how staining findings may be integrated with clinical examination and laboratory testing to guide patient assessment and management.
